Job summary

Molina Healthcare is hiring a Care Manager to support LTSS coordination and care management activities. You will collaborate with a multidisciplinary team to ensure member progress toward outcomes and to implement cost-effective care strategies across the continuum.

The role requires travel (25-40%), strong assessment skills, and experience with waiver services. You will engage with members, caregivers, physicians, and support networks to address complex needs and barriers to care.

Qualifications

• At least 2 years health care experience with LTSS or related field.
• Licensure/certification required only if mandated by state contract.
• Bachelor's degree may be required in some states.
• Valid driver’s license and reliable transportation for travel.
• Experience with community resources and diverse populations.
• Strong verbal and written communication skills.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment/disenrollment processes.
• Develops/implements care plans with members, caregivers, physicians, and networks.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Promotes integration of LTSS with behavioral health and community resources.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources; coordinates ICT meetings.
• Uses motivational interviewing to educate and motivate members.
• Assesses barriers to care and assists with psycho/social/financial obstacles.
• Identifies incidents and develops prevention plans; collaborates with care managers.
