Job summary

Molina Healthcare is seeking a Care Management/LTSS professional to support care coordination across the care continuum for high-need members in the United States. You will work with a multidisciplinary team to develop and monitor care plans, coordinate waiver services, and ensure timely assessments and interventions.

The role emphasizes proactive care coordination, collaboration with physicians and caregivers, and travel within the state to support member needs and program requirements.

Qualifications

• At least 2 years of health care experience with LTSS focus.
• LPN/LVN licensure required if state contract mandates it; active/unrestricted.
• Bachelor's degree may be required in some states/contractual requirements.

Responsibilities

• Completes comprehensive member assessments within regulated timelines (including in-person home visits).
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans with members and care network.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of behavioral health and LTSS services.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources.
• Facilitates ICT meetings for service approvals/denials.
• Uses motivational interviewing to support member change.
• Identifies barriers to care and coordinates psycho/social support.
• Identifies critical incidents and develops prevention plans.
• Travel 25-40% depending on state/contractual requirements.
