Job summary

Molina Healthcare is seeking a Care Manager to support LTSS activities and coordinate member care across the continuum. You will work with a multidisciplinary team to ensure high-need members achieve optimal outcomes.

The role involves developing care plans, monitoring progress, and facilitating access to home and community services, with travel to member homes as required. Strong organizational and communication skills are essential.

Qualifications

• At least 2 years of health care experience, including LTSS involvement.
• License/certification required only if state contract mandates it.
• Bachelor's degree may be required per state/contract.
• Valid driver’s license and reliable transportation for travel.
• Strong knowledge of community resources and social services.
• Ability to work independently across settings and with diverse populations.
• Excellent verbal and written communication skills.
• Experience with substance use disorders is a plus in some states.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits if required.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement care plans with the member, family, and care team.
• Monitor and adjust care plans based on progress and outcomes.
• Promote integration of services across behavioral health and LTSS.
• Assess medical necessity and authorize appropriate waiver services.
• Collaborate with ICT for service decisions and care coordination.
• Educate and motivate members using motivational interviewing techniques.
• Identify barriers and coordinate resources to address psychosocial, financial, and medical needs.
• Identify critical incidents and develop prevention plans.
• Travel up to 25–40% may be required.
