Molina Healthcare, Inc. is seeking a Care Management Support professional to coordinate long-term services and supports (LTSS) across multiple counties. You will work with a multidisciplinary team to ensure complete assessments, care plans, and timely authorization of waiver services.

The role involves travel to local areas and collaboration with physicians, caregivers, and care teams to optimize member outcomes and cost-effective care delivery.

Qualifications

• At least 2 years of health care experience including LTSS exposure.
• Clinical licensure and/or certification if required by state contract or regulation.
• Bachelor's degree in a health care field may be required in some states.
• Valid driver's license and reliable transportation for travel requirements.
• Ability to work with diverse populations and promote client-centered care.

Responsibilities

• Completes comprehensive member assessments within timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements member care plans with the care team and family.
• Monitors care plan effectiveness and documents outcomes.
• Promotes integration of LTSS with behavioral health and community resources.
• Assesses medical necessity and authorizes waiver services.
• Participates in ICT meetings for service decisions.
