Job summary

Molina Healthcare is seeking a care management professional to support LTSS activities and coordinate integrated member care across the continuum. You will collaborate with a multidisciplinary team to ensure progress toward outcomes and contribute to cost-efficient, high-quality care delivery.

Responsibilities include conducting assessments, enabling waiver services, developing care plans with stakeholders, and monitoring effectiveness. Some travel within the state is required.

Qualifications

• Minimum 2 years of health care experience including care management.
• Licensed clinical social work or equivalent licensure per state requirements.
• Valid driver’s license and reliable transportation for travel.
• Strong communication and coordination skills.
• Ability to work independently and with diverse populations.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and other health professionals.
• Monitor and adjust care plans to achieve outcomes.
• Facilitate interdisciplinary care team meetings for approvals and collaboration.
• Use motivational interviewing and Molina guidelines to educate and support members.
• Identify barriers to care and assist with psycho-social, financial, and medical issues.
• Travel 25-40% as required.
