Qualifications

• At least 2 years of health care experience with LTSS exposure.
• Clinical licensure/certification required only if state contract mandates.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans with members, families, and health professionals.
• Monitors care plan effectiveness and documents interventions.
• Promotes integration of services across behavioral health and LTSS.
• Assess for medical necessity and authorize waiver services.
• Collaborates with interdisciplinary care teams for approvals/denials.
• Educates and motivates members using Molina clinical guidance.
• Identifies barriers to care and coordinates resources.
• Recognizes critical incidents and develops prevention plans.
• Travel 25-40% may be required depending on state/contract.
