Job summary

Molina Healthcare is seeking a qualified care manager to support LTSS and care coordination for high-need members. You will collaborate with a multidisciplinary team to develop and implement care plans, monitor progress, and promote integrated services across behavioral health, LTSS, and community resources.

The role requires active licensure as an LCSW/LMSW/APSW/LPC/LPCC/LMFT and prior experience in care management or LTSS settings.

Qualifications

Valid licensure as an social worker (LCSW/LMSW/APSW/LPC/LPCC/LMFT, PhD/PsyD where applicable).
Experience in care management, managed care or LTSS settings preferred.
Ability to work with diverse populations and maintain professional relationships.

Responsibilities

Completes comprehensive member assessments within regulated timelines, including in-person home visits.
Facilitates waiver enrollment and disenrollment processes.
Develops and implements care plans with member input and care teams.
Monitors care plan effectiveness, documents interventions, and adjusts as needed.
Promotes service integration across behavioral health, LTSS and community resources.
Assesses medical necessity and authorizes waiver services; coordinates ICT meetings.
