Job summary

Molina Healthcare is seeking a skilled LTSS Supervisor to lead a multidisciplinary team delivering long-term services and supports. You will coordinate care across the continuum, ensure quality, and partner with leadership to meet regulatory obligations.

The role requires hands-on leadership, strong communication, and the ability to manage complex cases while optimizing member outcomes and care delivery efficiency.

Qualifications

• At least 5 years of experience in health care, including 2 years in LTSS/IHSS or equivalent.

Responsibilities

• Oversees team responsible for delivering LTSS member care.
• Provides direction to LTSS care coordination team and ensures timely delivery and regulatory compliance.
• Manages staff LTSS caseloads and assigns cases by complexity and licensure.
• Oversees use of electronic case management documentation in HIPAA-compliant manner.
• Arranges and facilitates staff training as needed.
• Identifies process improvements with leadership.
• Supports social service interventions for complex cases.
• Monitors team activities to support integrated care and individualized service plans.
• Assists with state-specific LTSS reporting requirements.
• Promotes collaboration with providers, county social workers, and families.
• Hires, trains, develops, and manages team; monitors performance.
• Local travel may be required.
