The role emphasizes staff development, process improvements, and collaboration with county social workers to sustain continuity of care for members across Barron, Douglas, Polk, and Washburn counties in Wisconsin.

Qualifications

• Minimum 5 years of healthcare experience, with 2+ years in LTSS/IHSS settings.
• Active licensure/certification as required by state contract or regulation.
• Strong knowledge of disability and senior access issues.
• Excellent written and verbal communication skills.

Responsibilities

• Oversee LTSS care coordination team and ensure timely delivery of quality services.
• supervise LTSS team, provide direction and ensure regulatory timelines are met.
• Manage staff caseloads and assign cases by complexity and experience.
• Ensure HIPAA compliant documentation and adherence to Molina processes.
• Lead staff training and drive process improvements with leadership.
