Qualifications

• At least 2 years of health care experience including LTSS.
• Licensure/certification required only if state contract mandates it.
• Bachelor's degree may be required in some states.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plan effectiveness and document interventions.
• Coordinate services across behavioral health and LTSS for continuity of care.
• Assess medical necessity and authorize waiver services.
• Collaborate with interdisciplinary care teams for approvals.
• Educate and motivate members using motivational interviewing.
• Identify barriers and provide care coordination for psycho/social/financial needs.
• Identify critical incidents and develop prevention plans.
• Travel may be required (25-40%) based on state contracts.
