Qualifications

• 2+ years of healthcare experience with LTSS or care management
• Clinical licensure/certification as required by state contract

• Valid driver's license and reliable transportation for travel

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits
• Facilitate waiver enrollment and disenrollment
• Develop and implement care plans with members and care network
• Monitor care plan effectiveness and document progress
• Promote integration of LTSS and behavioral health services
• Assess medical necessity and authorize waiver services
• Collaborate with interdisciplinary care team meetings
• Educate and motivate members during contacts
• Identify barriers to care and address psycho/social obstacles
• Travel 25-40% as required by state/contract
• Maintain professional relationships and communicate effectively
