Job summary

Ozarks Community Hospital in Springfield, Missouri seeks a Chronic Care Management Specialist to join the Medical Home team. The role includes traveling with the Long Term Care Team to nursing facilities around the area and providing direct and indirect patient care under supervision.

Requirements include a high school diploma, completion of a CNA/MA/LPN program, and a current BLS certification. A valid driver's license and background check are required throughout employment.

Qualifications

• Oversee Chronic Care Management enrollment, follow-up, documentation, and billing procedures.
• Communicate patient clinical condition with providers and team; coordinate with Long Term Care Team for nursing facility visits.

Ozarks Community Hospital (OCH), located in Springfield, Missouri is seeking a Chronic Care Management Specialist to join our Medical Home department. This position requires traveling with our Long Term Care Team to nursing facilities around the area.

Reports directly to Lead CCM and Medical Home Manager. Provides direct and indirect patient care in the office/clinic setting. Provides care that meets the psychosocial, physical, and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a registered nurse and/or physician, nurse practitioner or physician assistant and clinic manager. Maintains regulatory requirements, nursing and office/clinic policies, procedures, and standards. Communicates with providers and team members about patient's clinical condition, including results of diagnostic studies and symptomatology. Oversees Chronic Care Management including enrollment, follow-up, documentation, report running and billing procedures. This position requires traveling with our Long Term Care Team to nursing facilities around the area.

Requirements:

• High school diploma or equivalent.
• Successful completion of a Certified Nursing Assistant (CNA), Medical Assistant (MA), or Licensed Practical Nurse(LPN/LVN) program.
• Current BLS certification upon hire or BLS certification must be obtained during orientation period or at the next available scheduled class. Throughout employment, current BLS certification must be maintained by renewal during renewal period or at next available scheduled class.
• Valid Driver's License required. This requires a Motor Vehicle Driving Record Background Check to be completed during the onboarding new hire process and throughout employment.
