Job summary

Ozarks Community Hospital (OCH) in Springfield, MO is seeking a Chronic Care Management Specialist to join the Medical Home department. This role requires traveling with the Long Term Care Team to nursing facilities around the area, providing CCM enrollment, follow-up and documentation.

Responsibilities include coordinating care, managing billing procedures, and ensuring regulatory compliance under supervision of a RN/physician. A CNA/MA/LPN/LVN program completion and a current BLS are required.

Qualifications

• High school diploma or equivalent is required.
• Successful completion of a Certified Nursing Assistant (CNA), Medical Assistant (MA), or Licensed Practical Nurse (LPN/LVN) program.
• Current BLS certification or obtain during orientation.
• Valid Driver's License is required; driving record background check will be completed during onboarding.

Ozarks Community Hospital (OCH), located in Springfield, Missouri is seeking a Chronic Care Management Specialist to join our Medical Home department. This position requires traveling with our Long Term Care Team to nursing facilities around the area.

Reports directly to Lead CCM and Medical Home Manager. Provides direct and indirect patient care in the office/clinic setting. Provides care that meets the psychosocial, physical, and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a registered nurse and/or physician, nurse practitioner or physician assistant and clinic manager. Maintains regulatory requirements, nursing and office/clinic policies, procedures, and standards. Communicates with providers and team members about patient's clinical condition, including results of diagnostic studies and symptomatology. Oversees Chronic Care Management including enrollment, follow-up, documentation, report running and billing procedures. This position requires traveling with our Long Term Care Team to nursing facilities around the area.

Requirements:

• High school diploma or equivalent.
• Successful completion of a Certified Nursing Assistant (CNA), Medical Assistant (MA), or Licensed Practical Nurse(LPN/LVN) program.
• Current BLS certification upon hire or BLS certification must be obtained during orientation period or at the next available scheduled class. Throughout employment, current BLS certification must be maintained by renewal during renewal period or at next available scheduled class.
• Valid Driver's License required. This requires a Motor Vehicle Driving Record Background Check to be completed during the onboarding new hire process and throughout employment.
