Ozarks Community Hospital in Springfield, Missouri, is seeking a Long Term Care Licensed Practical Nurse (LTC LPN) to join our Medical Home department. The role provides direct and indirect patient care in partnering long-term care facilities, focusing on compassionate, quality nursing, interdisciplinary communication, and adherence to regulatory standards and facility policies.

The ideal candidate is a graduate of an accredited nursing program with current LPN licensure in Missouri and BLS

Ozarks Community Hospital (OCH), located in Springfield, Missouri is seeking a Long Term Care Licensed Practical Nurse (LTC LPN) to join our Medical Home department.

Provides direct and indirect patient care in partnering regional long term care facilities. Provides care that meets the psychosocial, physical, and general aspects of care. Meets the communication needs of patient and family. Provides care that reflects initiative and responsibility indicative of professional expectations, under the scope of licensure. Maintains regulatory requirements, nursing and facility policies, procedures, and standards. Communicates with physicians and team members about patient's clinical condition, including results of diagnostic studies and symptomatology.

Requirements:

• Graduate of an accredited school of nursing.
• Current Licensed Practical Nurse licensure in the state of practice.
• Minimum of one (1) year experience in the outpatient setting is preferred.
• Current BLS certification upon hire or BLS certification obtained during orientation period. Must maintain current BLS certification throughout employment.
