Job summary

Ozarks Community Hospital in Springfield, Missouri seeks a Chronic Care Management Specialist to join the Medical Home team. The role includes traveling with the Long Term Care Team to nursing facilities around the area and providing direct and indirect patient care under supervision.

Requirements include a high school diploma, completion of a CNA/MA/LPN program, and a current BLS certification. A valid driver's license and background check are required throughout employment.

Qualifications

• High school diploma or equivalent.
• Certified Nursing Assistant (CNA), Medical Assistant (MA), or Licensed Practical Nurse (LPN/LVN) program completed.
• Current BLS certification at hire or during orientation, and maintained thereafter.
• Valid driver's license with motor vehicle driving record background check during onboarding and employment.

Responsibilities

• Provide direct and indirect patient care in office/clinic setting under supervision.
• Oversee Chronic Care Management enrollment, follow-up, documentation, and billing procedures.
• Communicate patient clinical condition with providers and team; coordinate with Long Term Care Team for nursing facility visits.
