Ozarks Community Hospital in Springfield, Missouri is seeking a Long Term Care Licensed Practical Nurse (LTC LPN) to join our Medical Home department.

This role provides direct and indirect patient care across partnering long term care facilities, collaborates with physicians and the care team, and ensures care meets psychosocial, physical and regulatory requirements.

Qualifications

• Graduate of an accredited school of nursing.
• Current LPN licensure in Missouri.
• Minimum of one year outpatient experience preferred.
• Current BLS certification on hire or during orientation; maintain throughout employment.

Responsibilities

Ozarks Community Hospital (OCH), located in Springfield, Missouri is seeking a Long Term Care Licensed Practical Nurse (LTC LPN) to join our Medical Home department.

Provides direct and indirect patient care in partnering regional long term care facilities. Provides care that meets the psychosocial, physical, and general aspects of care. Meets the communication needs of patient and family. Provides care that reflects initiative and responsibility indicative of professional expectations, under the scope of licensure. Maintains regulatory requirements, nursing and facility policies, procedures, and standards. Communicates with physicians and team members about patient's clinical condition, including results of diagnostic studies and symptomatology.
