Job summary

Ozarks Community Hospital (OCH) in Springfield, Missouri is seeking a Long Term Care Licensed Practical Nurse (LTC LPN) to join our Medical Home department. The role focuses on direct and indirect patient care across partnering long term care facilities, aligned with OCH’s mission to provide compassionate, accessible health services.

The ideal candidate will be a graduate of an accredited school of nursing with active LPN licensure in Missouri and a minimum of one year outpatient experience

Qualifications

• Graduate of an accredited school of nursing.
• Current Licensed Practical Nurse licensure in the state of practice.
• Minimum of one (1) year experience in the outpatient setting is preferred.
• Current BLS certification upon hire or BLS certification obtained during orientation period. Must maintain current BLS certification throughout employment.

Responsibilities

• Provides direct and indirect patient care in partnering regional long term care facilities.
• Provides care that meets psychosocial, physical, and general aspects of care.
• Meets the communication needs of patient and family.
• Communicates with physicians and team members about patient's clinical condition, including results of diagnostic studies and symptomatology.
