Vitalia Mentor is hiring a Wellness Nurse Manager to lead the on-site wellness team in Mentor, OH. This part-time role supports residents with compassionate, person-centered nursing care during the 7:00 pm–7:00 am shift, with a $1,000 sign-on bonus.

The ideal candidate holds an LPN or RN license in good standing, can perform medication passes, document accurately, and promote safety and dignity for memory-impaired residents within regulatory standards.

Qualifications

• Current LPN or RN license in good standing in Ohio.
• Provide direct nursing care, including medication passes.
• Thorough knowledge of current care standards and regulations.
• EMR experience preferred but not required.

Responsibilities

• Lead Wellness team and floor coverage to ensure timely, person-centered care.
• Assist residents with call lights and urgent needs with urgency and compassion.
• Document resident assessments and medical records accurately.
• Maintain professional interactions while promoting safety and dignity.
