Job summary

Vitality seeks a compassionate LPN/LVN to join our Wellness team and promote safe, dignified care within our community. You will administer medications, monitor vital signs, collect specimens, and coordinate with physicians and the Wellness Director on service plans.

The role requires fluent English, current CPR certification, and an active license. You will supervise Personal Care Specialists and participate in onboarding and ongoing training to maintain a safe, professional environment.

Qualifications

• Must be at least 21 years old.
• Ability to communicate effectively in English, both verbally and in writing.
• Meet state health related requirements.
• Maintain current CPR certification if LPN/LVN.
• Maintain current professional license if LPN/LVN.

Responsibilities

• Administer or assist residents to self-administer medications and treatments.
• Measure vital signs and weight.
• Collect, store, and transport specimens for laboratory studies.
• Communicate with physicians via telephone, fax, and physician forms.
• Implement physician orders and document in resident charts and nursing notes.
• Supervise Personal Care Specialists to ensure daily duties are completed.
• Assist in development and revision of service plans and report resident needs to Wellness Director.
