Job summary

Vitality Hospice & Palliative Care is seeking a Per Diem Licensed Practical Nurse (LPN) to deliver compassionate, patient-centered hospice care in patients' homes or care facilities. The role emphasizes autonomy and flexible weekend scheduling.

Under the supervision of an RN Case Manager, you will perform assessments, manage symptoms, assist with ADLs, educate families, document visits, and participate in optional on-call rotations. Local travel and mileage reimbursement are provided.

Qualifications

• Active and unencumbered LPN license in NJ.
• Minimum 1 year of nursing experience; hospice experience a plus.
• Knowledge of hospice philosophy and care for terminally ill patients.
• Strong assessment, communication, and documentation skills.
• Ability to work independently and manage time effectively.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Provide skilled hospice nursing care in accordance with the patient’s individualized plan of care.
• Perform nursing assessments and promptly report changes in patient condition.
• Support pain and symptom management to promote comfort and quality of life.
• Assist patients with activities of daily living (ADLs) as needed.
• Educate patients, families, and caregivers on care needs and self-care strategies.
• Accurately document visits and care provided in a timely manner.
• Collaborate with the interdisciplinary hospice team to ensure coordinated, holistic care.
• Participate in on‑call rotation as available (optional, based on per diem needs).
