Vitality Hospice & Palliative Care is seeking a dedicated Per Diem Licensed Practical Nurse (LPN) to provide high-quality, patient-centered hospice care in patients' homes or care facilities. This weekend, per diem role offers flexibility, autonomy, and meaningful one-on-one patient care.

Under RN supervision, you will assess patients, manage symptoms, educate families, and document care. Local travel is required with mileage reimbursement; participate in on-call rotation as needed.

Qualifications

• Active and unencumbered LPN license in the state of practice.
• Minimum 1 year of nursing experience; hospice experience a plus.
• Knowledge of hospice philosophy and end-of-life care.
• Strong assessment, communication, and documentation skills.
• Ability to work independently and manage time effectively.

Responsabilités

• Provide skilled hospice nursing care per patient plan of care.
• Perform nursing assessments and report changes promptly.
• Support pain and symptom management for comfort.
• Assist patients with activities of daily living as needed.
• Educate patients and families on care needs and self-care.
• Document visits and care provided timely.
• Collaborate with interdisciplinary hospice team for holistic care.
• Participate in on-call rotation as available.
