Job summary

Hopkins Restorative Care Center is seeking an organized LPN Unit Manager / Health Unit Coordinator to join our skilled nursing team. This role combines unit management, clinical oversight, staff coordination, scheduling support, and HUC responsibilities.

The successful candidate will oversee daily unit operations, coordinate admissions/discharges, and mentor nurses and CNAs while ensuring accurate documentation and resident-focused care. Strong leadership and organizational skills are essential.

Qualifications

• Current LPN license in good standing.
• Long‑term care or skilled nursing experience preferred.
• Previous Unit Manager, Charge Nurse, HUC, or nursing leadership experience preferred.
• Strong clinical judgment and resident‑assessment skills.
• Excellent organizational and time‑management abilities.
• Strong communication and leadership skills.
• Ability to coach staff and hold team members accountable in a professional manner.
• Ability to manage multiple priorities in a fast‑paced environment.
• Comfortable working with electronic health records and computerized systems.
• PointClickCare experience preferred.
• Knowledge of long‑term care regulations and survey expectations is a plus.
• Dependable, approachable, team‑oriented, and willing to assist where needed.

Responsibilities

• Provide daily leadership and oversight of the nursing unit.
• Support, coach, and assist nurses and CNAs with resident care and follow-through.
• Monitor resident changes, assessments, documentation, orders, and care plans.
• Coordinate admissions, discharges, appointments, labs, pharmacy, and provider communication.
• Assist with nursing schedules, staffing needs, call‑ins, and open shifts.
• Complete resident rounding and follow up on incidents, falls, skin concerns, and other clinical issues.
• Maintain organized nursing records, supplies, equipment, and unit communication.
• Assist with audits, staff education, QAPI, infection prevention, and survey readiness.
• Support new employee orientation and ongoing staff accountability.
• Provide direct LPN nursing care as needed and within scope of practice.

We are seeking an organized, dependable, and resident-focused LPN Unit Manager / Health Unit Coordinator (HUC) to join our skilled nursing team.

This unique position combines unit management, clinical oversight, staff coordination, scheduling support, and Health Unit Coordinator responsibilities. It is ideal for an experienced LPN who enjoys working directly with residents and staff while also keeping the unit organized, accountable, and running smoothly.

What You’ll Do

The LPN Unit Manager / HUC Coordinator will work closely with the Director of Nursing, nursing leadership, nurses, CNAs, providers, and interdisciplinary team to oversee the daily operation of the assigned nursing unit.

Key Responsibilities

• Provide daily leadership and oversight of the nursing unit

• Support, coach, and assist nurses and CNAs with resident care and follow-through
