Job summary

Sweetwater Care in Dillon, MT seeks an LPN with long-term care experience to provide nursing care within scope of practice and ensure patient needs are met according to standards and physician orders. The role includes participating in quality improvement, educating families and patients, and coordinating with physicians.

Experience with POINT CLICK CARE is required. Candidates must be licensed in Montana and have some nursing management experience; this full-time on-site position offers

Qualifications

Responsibilities

• Provide nursing care within scope of practice and meet patient needs per standards, physician orders, policies, and guidelines.
• Demonstrate knowledge and use of infection control practices.
• Participate in quality improvement and patient education.
• Coordinate with physicians and other departments; document changes and orders.
• Meds and treatments administered timely per facility policy; rounds with physicians when needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LPN in state
• One-year prior nursing management experience preferred
• Must have long term care experience.
• Must have electronical charting system experience with POINT CLICK CARE.
