Job summary

Sweetwater Care in Lakewood, CO seeks an experienced LPN with nursing management background to oversee patient care within LTC settings. You will ensure timely medication administration, coordinate treatments, and support physicians during rounds in a compliant, patient-centered environment.

Candidate must hold active LPN licensure in Colorado, have LTC experience, and possess proficiency with POINT CLICK CARE. Prior management experience preferred and you will work full-time on-site.

Qualifications

• Must have current LPN licensure in the state.
• One-year prior nursing management experience preferred.
• Must have long-term care experience.
• Experience with electronic charting (POINT CLICK CARE) is required.

Responsibilities

• Provides nursing care within scope of practice and in accordance with standards and physician orders.
• Implements Quality Improvement and Quality Life programs as applicable.
• Educates patients and families as needed.
• Administers medications and treatments in a timely manner per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Occasionally conducts rounds with physicians.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LPN in state
• One-year prior nursing management experience preferred
• Must have long term care experience.
• Must have electronical charting system experience with POINT CLICK CARE.
