Job summary

Sweetwater Care in Texas is seeking an LPN with nursing management experience to lead clinical operations in a long-term care setting. You will ensure patient care compliance, coordinate with physicians, and oversee medication administration and charting processes.

The role requires licensure as an LPN in Texas, prior LTC leadership, and experience with electronic medical records such as POINT CLICK CARE. This is a full-time, on-site position offering competitive compensation.

Qualifications

• Currently licensed as LPN in state.
• One-year prior nursing management experience preferred.
• Must have long term care experience.
• Must have electronic charting system experience with POINT CLICK CARE.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in implementing the Quality Improvement Program.
• Uses the Quality Life Manual for patient identification, placement, and monitoring in appropriate programs.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders, if allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
