Job summary

Sweetwater Care is seeking a licensed practical nurse with leadership skills to oversee nursing care in a long-term care setting in Houston, TX. You will ensure care meets standards, coordinate with physicians, and guide staff to deliver timely, quality care.

Responsibilities include medication administration, patient education, and rounds with physicians, with participation in quality improvement. New grads are welcome; LTC and PointClickCare experience are valued.

Qualifications

• Licensed as LPN in state.
• 1 year nursing management experience preferred.
• New graduates encouraged to apply.
• Experience in long-term care.
• Experience with PointClickCare charting.

• Participates in implementing the Quality Improvement Program.
• Uses the Quality Life Manual for patient identification, placement, and monitoring.
• Notifies other Departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Mans rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.

• Understands, participates and assists in implementing the Quality Improvement Program.

• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.

• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.

• Conducts family and patient education.

• Demonstrates the ability to administer medications timely and according to facility policy.

• Demonstrates the ability to administer treatments timely and according to facility policy.

• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act

• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LPN in state
