Job summary

Sweetwater Care in Montana is seeking an experienced LPN to provide nursing leadership and ensure patient care meets standards and physician orders within facility policies. This role requires timely medication administration and coordination with the healthcare team.

You will conduct patient and family education, participate in quality improvement, infection control, and ensure accurate transcription and execution of physician orders as allowed by the Nurse Practice Act.

Qualifications

• Currently licensed as LPN in state.
• Must have long term care experience.
• Must have electronic charting system experience with POINT CLICK CARE.
• One-year nursing management experience preferred.

Responsibilities

• Provides nursing care within scope of practice to meet patient needs according to standards of practice, physician orders, center policies and procedures, and state/local guidelines.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.
• Notifies other departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Supports quality improvement initiatives and infection control practices.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.
