Job summary

Sweetwater Care in Lakewood, CO seeks an LPN with nursing management experience to oversee resident care and coordinate with physicians, families, and other departments.

The role requires LT care experience, an active LPN license, and proficiency with electronic charting (POINT CLICK CARE). You will participate in QA activities and education while ensuring timely medications and treatments.

Qualifications

• Licensed LPN in Colorado.
• Prior nursing management experience preferred.
• Must have long-term care experience.

Responsibilities

• Provides nursing care within scope of practice.
• Participates in Quality Improvement Program.
• Utilizes Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, admissions, discharge).
• Conducts family and patient education.
• Administers medications timely according to policy.
• Administers treatments timely according to policy.
• Receives, transcribes, and carries out physician orders when allowed by Nurse Practice Act.
• Makes rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LPN in state
• One-year prior nursing management experience preferred
• Must have long term care experience.
• Must have electronical charting system experience with POINT CLICK CARE.
