Job summary

BAYADA Home Health Care is seeking LPNs for the Transition Team to deliver high-quality home care for patients across pediatric and adult populations in Philadelphia. You will help transition care from hospital settings to home care and train the BAYADA team to ensure continuity and safety.

Qualifications

• Have a current license as a Licensed Practical Nurse
• NICU, PICU, ICU, or home care pediatric trach/vent experience required
• Achieve a passing score on high-tech Adult and Pediatric simulations.
• Hold current CPR certification
• Ability to work in a teaching setting to prepare and educate the BAYADA care team
• Availability to commit to an ongoing, consistent schedule
• Ability to travel throughout Philadelphia County

Job description

We have immediate opportunities on our Transition Team for Licensed Practical Nurses / LPNs interested in helping infants, children, and adults receive complex care at home in Philadelphia, PA! This team will support the transition of care from the hospital or from our HICU® Health Services team to ongoing home care services. This role offers a unique opportunity to partner with CHOP and learn how to care for clients with rare and complex diseases. As part of the Transition Team, you will will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.

Present openings are for overnight shifts Monday through Sunday!

LPNs at BAYADA earn a salary of $30-$42 per hour!

Rate is based on qualifications and experience for the Transition Team role.

Transition Team rates: $40-$42

Standard rates: $30-$35
