Job summary

BAYADA Home Health Care in Pennsylvania is seeking Licensed Practical Nurses (LPNs) to join our Transition Team, delivering complex care at home for infants, children, and adults across Chester, Delaware, and Montgomery counties. This is a full-time 40-hour-per-week role with day and overnight shifts.

You will support transitions from hospital to home care, provide hands-on clinical care, and train the ongoing BAYADA care team.

Qualifications

• Licensed Practical Nurse with active LPN license and CPR certification.
• Experience in NICU/PICU/ICU or pediatric home care trach/vent required.
• Ability to travel across Chester, Delaware and Montgomery counties.
• Willingness to commit to a 40-hour weekly schedule, including weekends.

Responsibilities

• Deliver quality home health care to infants, children, and adults.
• Provide training to BAYADA care teams and ongoing staff.
• Support the transition of care from hospital to home care.
• Collaborate with hospital and home care teams to ensure smooth handoffs.

Job description

We have immediate opportunities on our Transition Team for Licensed Practical Nurses / LPNs interested in helping infants, children, and adults receive complex care at home throughout Chester County, Delaware County and Montgomery County! The role is FT – 40 hours per week. This team will support the transition of care from the hospital team to ongoing home care services. This role offers a unique opportunity to learn how to care for clients with rare and complex diseases. As part of the Transition Team, you will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.

Present openings are Days & Overnight shifts (Monday through Sunday).

Qualifications for Transition Team Licensed Practical Nurses / LPNs

• NICU, PICU, ICU, or home care trach/vent experience required
• Pediatric trach/vent experience required
• Achieve a passing score on high-tech Adult and Pediatric simulations
• Hold current CPR certification
• Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team
• Availability to commit to an ongoing, consistent schedule of 40 Hours per week
• Flexibility to cover weekends - at least four weekend shifts per month
• Ability to travel throughout Chester County, Delaware County and Montgomery County (only southern and western parts of MontCo)

Rate is based on qualifications and experience for the Transition Team role.

Transition Team rates: $40-$42

Standard rates: $30-$35
