Job summary

BAYADA Home Health Care is seeking Transition Team Registered Nurses to help infants, children, and adults receive complex care at home in Philadelphia, PA. You will move patients from hospital settings to BAYADA home care, partnering with CHOP and teaching the BAYADA team for continuity of care.

Qualifications

• Must hold a current RN license.
• NICU/PICU/ICU or home care vent experience required.
• Pass a high-tech Adult and Pediatric simulations.
• Current CPR certification.
• Ability to work in a teaching setting to educate the BAYADA care team.
• Availability to commit to an ongoing, consistent schedule.
• Ability to travel throughout Philadelphia County.

We have immediate opportunities on our Transition Team for Registered Nurses / RNs interested in helping infants, children, and adults receive complex care at home in Philadelphia, PA! This team will support the transition of care from the hospital or from our HICU® Health Services team to ongoing home care services. This role offers a unique opportunity to partner with CHOP and learn how to care for clients with rare and complex diseases. As part of the Transition Team, you will will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.

Present weekend openings are overnights (Friday through Sunday nights) and day shifts (Saturday and Sunday days).

Qualifications for Transition Team Registered Nurses / RNs:

• Have a current license as a Registered Nurse / RN
• NICU, PICU, ICU, or home care trach/vent experience required
• Achieve a passing score on high-tech Adult and Pediatric simulations.
• Hold current CPR certification
• Ability to work in a teaching setting to prepare and educate the ongoing BAYADA care team
• Availability to commit to an ongoing, consistent schedule
• Ability to travel throughout Philadelphia County

Rate is based on qualifications and experience for the Transition Team role.
