Job summary

BAYADA Home Health Care is seeking an LPN to join our overnight Skilled Nursing team serving adult clients in the Horsham area and nearby communities. You will provide one-to-one client care in private duty home settings, traveling to client homes and documenting care per physician orders.

The role emphasizes compassionate, high-quality care with support from Clinical Managers and office staff, and offers comprehensive benefits and ongoing training opportunities.

Qualifications

• Graduation from an accredited nursing program.
• A current LPN license in good standing in the state.
• Ability to perform home visits with client in private duty setting.

Responsibilities

• Travel to one client near you for overnight shifts.
• Follow up with, execute, and properly document doctors' orders.
• Perform assessments and monitor clients' conditions in home settings.
• Document observations, interventions, and evaluations.

Qualifications for a private duty LPN:

• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• A current nursing license in good standing in the state
