Job summary

Signature HealthCARE is seeking a dedicated LPN/LVN in Memphis, TN to provide direct nursing care, administer medications, perform wound care, and support residents with ADLs with dignity and safety.

You will supervise nursing assistants and medication aides during shifts, coordinate admissions, and ensure accurate documentation and care plan updates in collaboration with the interdisciplinary team. Weekend availability, strong long-term care experience, CPR/BLS preferred.

Qualifications

• Active LPN/LVN license in the state of practice.
• Minimum 1 year of clinical nursing experience, preferably in long-term care.
• Experience in a supervisory or charge nurse role preferred.
• Strong documentation and medication administration skills.
• Familiarity with electronic health records (EHR).

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while ensuring safety and dignity.
• Supervise nursing assistants and medication aides during assigned shifts.
• Monitor residents and report changes to the physician and care team.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities and regulatory surveys.
