Job summary

Signature HealthCARE is seeking a dedicated LPN/LVN to join our Memphis facility for a Monday-Friday 3p-11p shift. You will provide direct nursing care, manage medications, and support residents with daily activities in a senior care environment.

The role requires an active LPN/LVN license in the state, and familiarity with EHRs and Microsoft Office. Prior supervisory experience and knowledge of long-term care regulations are a plus.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with EHR systems; proficient with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physician of changes.
• Develop and review individualized resident care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
