Job summary

Signature HealthCARE Portland Rehab & Wellness Center in Portland, TN is seeking an LPN/LVN to provide direct nursing care, including medications and wound care, while supervising nursing staff on the shift. You will monitor resident status, contribute to care plans, and ensure documentation accuracy.

The role requires an active LPN/LVN license in TN, 1+ year of nursing experience, and familiarity with EHR systems. Strong communication skills and ability to work independently are essential.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Supervisory or charge nurse experience preferred.
• Familiarity with electronic health records (EHR) systems.
• Effective verbal and written communication skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the shift in accordance with policies.
• Monitor and report changes in residents' status to physician and family as needed.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
