Job summary

Signature HealthCARE of Memphis is seeking an LPN/LVN to provide direct nursing care in a 140-bed facility, including medication administration and wound care. You will supervise nursing assistants, coordinate care with the interdisciplinary team, and ensure safety and comfort for residents.

The role emphasizes regulatory compliance, accurate documentation, and effective communication. A valid state license, CPR/BLS, and LTC experience are preferred.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Proficiency with Microsoft Office and EHR systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the assigned shift in accordance with facility policies.
• Monitor and observe residents’ physical and emotional status; notify the attending physician of any change in condition.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
