Signature HealthCARE – Chapel Hill is seeking an LPN/LVN to join our compassionate nursing team. You will provide direct nursing care, administer medications, and supervise nursing assistants during your shift.

The role emphasizes ADLs, safety, accurate documentation, and coordination of resident admissions, transfers, and discharges with the interdisciplinary team. An active LPN/LVN license in the state is required; CPR/BLS is preferred.

Formación

• Current, active LPN/LVN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with electronic health records (EHR) systems.

Responsabilidades

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the team.
• Complete accurate and timely clinical documentation per policy.
• Coordinate resident admissions, transfers, and discharges according to procedures.
• Participate in quality assurance, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
