Job summary

Northgate Park in Cincinnati, OH seeks a licensed practical nurse (LPN) to lead the shift and supervise RCAs/MTs, coordinating resident care and delegating assignments. You will administer medications and monitor safety, following MAR procedures and residence policies.

Qualifications

• Graduate of accredited school of nursing and current LPN license in the state of work.
• Minimum of two years nursing experience, preferably in Assisted Living or dementia care.
• Ability to read, write and speak English; physically able to lift up to 40 pounds.
• Completion of drug testing and criminal background check.

Responsibilities

• Supervise shift and coordinate resident care; assign tasks to RCAs and MTs.
• Administer medications according to physician orders and MAR.
• Document medications and care activities in MAR; ensure accuracy.
• Conduct shift reports and maintain medication cart security.
• Orient and train new associates; evaluate skills and provide feedback.
• Respond promptly to emergencies and ensure safety protocols.
• Monitor resident charts and report changes to physician and responsible party.
