The LPN must hold a current state license, CPR certification, and have at least 1 year of senior-care experience, with strong teamwork and computer skills to document care and coordinate admissions as needed.

Qualifications

• Must have a Licensed Practical Nurse license current in the state where practicing.
• CPR certification is required.
• English proficiency to perform duties and communicate in emergencies.
• 1+ year experience in senior care or related field.
• Technical competency includes basic computer skills.

Responsibilities

• Responsible for medication administration following regulations and guidelines.
• May handle admissions assessments and documentation for new residents.
• Complete acute and long-term care or service plans.
• May track clinical information (e.g., wounds, infection control) under direction.
• Provide health-related consultations and communicate changes to physicians/families.
• Notify physicians and families of health changes and document properly.
• Respond quickly to emergencies and coordinate with staff.
• Maintain communication with departments and staff.
• Provide educational sessions for residents and staff.
