Angels Care Home Health is seeking an LPN Scheduler to manage patient visit scheduling using Home Care Home Base. You will coordinate care with management and the patient interdisciplinary team, ensuring timely documentation and clear communication with patients, staff, and referral sources.

Respond to agency needs with professionalism and creativity. The role requires 1 year of home health scheduling, strong computer and typing skills, and knowledge of Medicare regulations.

Qualifications

• Minimum 1 year of home health scheduling experience.
• Excellent computer skills required.
• Excellent interpersonal and organizational skills.
• Typing speed 40-50 WPM.
• Knowledge of Medicare regulatory guidelines.
