Job summary

Angels Care Home Health in Omaha, NE is seeking an LPN Scheduler to process scheduling of nursing visits using the Home Care Home Base system. You will document patient schedules promptly and coordinate care with management and the interdisciplinary team.

The role requires at least 1 year of home health experience, strong computer, interpersonal, and organizational skills, and a valid LPN license along with a driver's license.

Qualifications

• Minimum 1 year of home health experience.
• Graduate of an accredited school of practical/vocational nursing.
• Current LPN license and driver’s license.
• Reliable transportation and valid auto liability insurance.

Responsibilities

• Schedule and document patient nursing visits in Home Care Home Base.
• Coordinate care with management and interdisciplinary team.
• Communicate effectively with patients, staff, other healthcare professionals and referral sources.
• Respond to agency and patient needs professionally and proactively.
