Job summary

Aspire Senior Living New Florence is seeking a compassionate LPN/RN MDS Coordinator to join our resident-focused care team in Missouri. Coordinate MDS assessments, develop individualized care plans, and collaborate with licensed nurses and other professionals to ensure high-quality care for our residents.

You will maintain accurate records, participate in quality improvement, and educate families about resident care and services. Missouri LPN or RN license required; LTC experience preferred.

Qualifications

• Current LPN or RN license in Missouri (LPN or RN)
• Experience in long-term care or related field
• Strong knowledge of MDS and RAI processes
• Strong organizational and communication skills

Responsibilities

• Coordinate the resident assessment process, including scheduling and conducting MDS assessments, and developing individualized care plans
• Identify individualized care needs and communicate with the interdisciplinary care team to develop and implement comprehensive care plans
• Collaborate with licensed nurses, therapists, and other healthcare professionals to ensure the delivery of high-quality care to our residents
• Maintain accurate and up-to-date records of resident care, including assessment results, care plans, and progress notes
• Participate in facility-wide quality improvement initiatives to promote resident-centered care and improve resident outcomes
• Provide education and support to family members and caregivers on resident care and services
