Aspire Senior Living is seeking an experienced MDS Coordinator (LPN or RN) to join our nursing team in Missouri. The role focuses on coordinating MDS assessments, care plans, and documentation to ensure high-quality resident care.

Responsibilities include collaboration with staff and care teams, developing individualized plans, timely documentation, and participation in quality improvement initiatives. A $4,000 sign-on bonus is available for full-time hires.

Qualifications

• Current LPN or RN licensure in Missouri.
• Experience in long-term care or senior living preferred.
• Excellent communication and organizational skills.

As an integral member of our nursing team, we are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to serve as our MDS Coordinator. In this critical role, MDS Coordinator will work closely with our nursing staff, healthcare providers, and residents to ensure the highest quality of care and support.

Responsibilities

• Coordinate the maintenance and completion of Minimum Data Set (MDS) assessments, care plans, and documentation for our residents
• Collaborate with nursing staff, healthcare providers, and residents to identify and address changes in resident conditions and needs
• Develop and implement individualized care plans that meet the unique needs of our residents
• Ensure accurate and timely completion of MDS assessments and care plans
• Maintain accurate and up-to-date records and reports
• Participate in quality improvement initiatives to enhance the overall quality of care and services

Requirements

• Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensure in the state of Missouri
• Experience in a long-term care or senior living setting preferred
• Strong clinical knowledge and skills in geriatric care
• Excellent communication, interpersonal, and organizational skills
• Ability to work effectively in a fast-paced environment and prioritize tasks and responsibilities
