Aspire Senior Living Pleasant Hill is seeking a dedicated LPN/RN MDS Coordinator to join our team. We are a premier resident-focused care provider, committed to delivering high-quality personal care in a warm and welcoming atmosphere.

You will coordinate and implement MDS assessments, collaborate with the interdisciplinary team, maintain accurate records, and contribute to quality improvement while educating staff through trainings and programs.

Qualifications

• Current license as LPN or RN in Missouri
• Experience in long-term care or related field preferred
• Strong understanding of MDS assessment and care planning principles
• Ability to work in a fast-paced environment and manage multiple responsibilities
• Excellent communication and problem-solving skills
• Ability to maintain confidentiality and handle sensitive information with discretion

Responsibilities

• Coordinate and implement MDS assessments
• Collaborate with Interdisciplinary Team to develop care plans
• Provide exceptional customer service to residents, families, and staff
• Maintain accurate records and streamline processes with technology
