Job summary

Aspire Senior Living New Florence is seeking a dedicated and compassionate MDS Nurse Coordinator to join our team. You will play a vital role in coordinating assessments and developing individualized care plans to ensure the highest quality care for residents.

Responsibilities include submitting MDS assessments, collaborating with the care team, and educating residents, families, and staff on the MDS process while maintaining compliance with regulations.

Qualifications

• Active LPN or RN license in Missouri.
• Strong understanding of long-term care regulations.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work in a fast-paced environment and prioritize multiple tasks.
• Strong analytical and critical thinking skills.

Responsibilities

• Coordinate and manage the assessment and care planning process for residents, including the development and implementation of Individualized Care Plans.
• Complete and submit accurate MDS assessments, Care Plans, and other required documentation.
• Collaborate with interdisciplinary team members to identify and meet the unique needs of each resident.
• Provide education and support to residents, families, and staff on the MDS process and care planning.
• Stay up-to-date with regulatory requirements and industry best practices to ensure compliance.
