Qualifications

• Must hold a registered nurse (RN) or licensed practical nurse (LPN) license in Massachusetts.
• Experience in long-term care, memory care, or sub-acute care preferred.
• Knowledge of MDS assessments and case management practices.
• Strong communication and teamwork skills.

Responsibilities

• Assess resident health problems and needs; develop nursing care plans.
• Administer medications and treatments as prescribed.
• Monitor resident progress; respond to changes in condition.
• Collaborate with multidisciplinary teams to provide comprehensive care.
• Educate residents and families about health conditions and discharge plans.
• Maintain HIPAA-compliant documentation and accurate records.
• Support in specialized areas such as PT, OT, SUD, pain management.
