Royal Health Group is seeking an MDS Nurse to join our family-owned skilled nursing facilities in Falmouth, MA. The role centers on MDS coordination, care planning, and ensuring compliance with state and federal regulations.

You will collaborate with nursing and clinical teams to deliver high-quality resident care, manage Medicare/MDS timelines, and support memory care and sub-acute services. Competitive weekly pay and insurance benefits start on day one.

Qualifications

• Active Registered Nurse license in the state where you practice.
• Experience with MDS processes and long-term care documentation.
• Familiarity with PCC EMR and Medicare/Medicaid requirements.

Responsibilities

• Facilitate weekly Medicare meetings and communicate skilled coverage criteria clearly.
• Transmit MDS to CMS and verify final validation reports for errors.
• Complete resident interviews and PPS MDS assessments within regulatory timeframes.
• Develop and update Interdisciplinary Care Plans and MDS assessments.
• Review and participate in invoices for skilled patients under billing rules.
• Maintain PPS schedule and set MDS ARDs within the MDS software.

As an MDS Nurse you will play a key role in providing exceptional long-term care, sub-acute care, and memory care. Your strong clinical skills and commitment to resident-centered care will help us meet the evolving needs of those we serve.

RESPONSIBILITIES

• Facilitates weekly skilled Medicare meetings and provides clear communication regarding skilled coverage criteria.
• Transmits MDS to CMS and follows up to validate any errors or messages revealed on final validation reports.
• Completes all resident Interviews and PPS MDS assessments within regulatory time frames including CAAs.
• Drives the development of the Interdisciplinary Care Plan and updates subsequent MDS Assessment.
• Participates and reviews invoices as indicated for skilled patients under consolidated billing.
• Maintains a current PPS schedule while setting all PPS MDS ARDs within time frames in the MDS software program.
• Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations.
• Participate in case management activities to ensure continuity of care.
• Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc..
• Collaborate with healthcare team members to deliver comprehensive resident care in our facilities.
• All other duties as assigned.

SKILLS

• Registered Nurse in the State
• EMR: PCC (Point Click Care)
• Proficiency in memory care techniques and protocols.
• Knowledge of MDS (Minimum Data Set) assessments for long-term care facilities.
• Familiarity with operations in Skilled Nursing, Sub-Acute, and Dementia Units.
• Strong background in case management practices.
• Ability to work effectively in a medical office environment.
• Knowledge of HIPPA, and state and federal regulations.
• Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.
• Strong organizational skills to manage multiple tasks efficiently.
