Qualifications

• Licensed Registered Nurse or Licensed Practical Nurse in Massachusetts.
• Experience with long-term care, sub-acute, or memory care.
• Proficiency with PCC EMR and MDS assessments.

Responsibilities

• Assess resident health needs and develop nursing care plans.
• Administer medications and treatments as prescribed.
• Monitor progress and respond to changes in condition, collaborating with the team.
• Educate residents and families about health conditions and discharge plans.
• Maintain HIPAA-compliant records and participate in case management.
