Job summary

Fairlawn Haven is seeking a dedicated LPN or RN for full-time 3rd shift (7 PM–7:30 AM) to monitor resident health status and care plans across the Care Center, Rehab, Assisted Living, and Long-Term Care neighborhoods. You will lead and support STNA staff in a collaborative, patient-focused environment.

Requirements include graduation from an accredited nursing program, current LPN/RN license in good standing, at least 1 year of long-term care experience preferred, and strong communication,

Qualifications

• Graduate of an accredited Nursing Program.
• Current LPN/RN nursing license, in good standing with the State licensing board;
• Minimum of one (1) year Long Term Care experience, preferred;
• Ability to maintain confidentiality, and honor Resident’s Rights;
• Must be able to communicate effectively with personnel, residents, family members, visitors, government agencies/personnel and the general public;
• Must have excellent attendance and work history;

Responsibilities

• Monitor health status of resident care plans across the Care Center.
• Assist Rehab, Assisted Living, Special Care, and Long-Term Care neighborhoods.
• Lead and support STNA staff as needed.
• Collaborate with interdisciplinary team to ensure quality care.
• Respond to changes in resident conditions promptly and professionally.
