Job summary

Fairlawn Haven in Archbold, OH is seeking a dedicated LPN/RN for a full-time, hybrid schedule with 12-hour shifts. The role focuses on assessing residents’ health and coordinating care across Rehab, Assisted Living, Special Care, and Long-Term Care neighborhoods.

Responsibilities

• Assist Rehab, Assisted Living, Special Care, and Long-Term Care neighborhoods.
• Lead and collaborate with LPNs and STNAs, with support from Clinical Managers.
• Maintain confidentiality and uphold Residents’ Rights.

Full Time (Hybrid Schedule)

12-hour shifts (Weekend is 1st shift, Weekday 3rd shift)

Includes holiday and weekend rotation

An LPN/RN will focus on assessing resident’s health and treatment throughout the Care Center. Assisting our Rehab, Assisted Living, Special Care, and the Long-Term Care neighborhoods. Candidate will lead and work as a team with LPNs and STNAs, with the support of Clinical Managers. Ideal candidate should have good assessment skills, the ability to work effectively in a changing environment, be a team player, possess a positive attitude, and be self-motivated.

QualificationsQualifications and/or Skills Required:

• Graduate of an accredited Nursing Program;
